New Patients: We have now converted to using an electronic medical records (EMR) system called Power2Practice.  If you have not spoken to our office staff about gaining access to your own Power2Patient Portal, please call our office.  It is through this EMR that you will fill out your new patient intake, sign consents and be able to message back and forth with your doctor.  Please note that you do not also need to fill out the intake form below once you have completed your online intake, however, if you have not read our welcome letter, signed your consent to treat or need to sign a request of records to have records forwarded to our clinic from another provider, you may print the forms below.

Return Patients: We will also be adding you to our electronic medical records (EMR) system called Power2Practice.  Our office staff will reach out to you before your follow-up appointment to get you access to your own Power2Patient Portal.  We ask that you too fill out your patient intake and sign the assigned documents.  If you have any questions, please call our office.  This new system allows our providers access to your chart at both our locations and allows you to securely send messages with our providers and support staff.
